Your First Visit
We ask that you arrive 10 minutes before your scheduled appointment to fill out a new patient form. This will allow your visit to run more smoothly. To save time, you have the option to download and complete the new patient form below.
Initial Examination
Your dentist will review your medical and dental history and then complete a comprehensive dental exam. The exam will consist of an evaluation of your gums, teeth, bite, and jaw, an oral cancer screening and any required X-rays. A fluoride rinse may also be administered to help strengthen your teeth and lower your risk of tooth decay.
Treatment Plan
Your dentist will develop a long-term dental treatment plan customized to your needs based on your examination, dental history, and oral health goals.
Insurance & Payment
At Cityplace Dental Group, we want to help make your dental experience as seamless as possible.
We do our best to help you navigate our options for payment and insurance so you can focus on more important things, like restoring your oral health and achieving your dental goals.
Insurance Claims
For your convenience, we can direct bill your insurance company for you and our administrative team is happy to assist you if you need help understanding your insurance plan more thoroughly.
Payment Methods
The full cost of your dental services and treatment must be paid at the time of your appointment. At Cityplace Dental Group, we are able to accept payment by cash, debit, Visa and Mastercard.

Yes, free on-site parking is available if your appointment is longer than two hours. Please note that we offer validation for the attached Cityplace parkade (Lot 4) only.
